Southwest Louisiana Contractors Safety Performance Recognition Program

1. Contractors must have a current PQF, and a current PSM Audit on file with the Southwest Louisiana PQF/Audit program, managed by the Safety Council of Southwest Louisiana. Contact us at audits@safetyally.org to arrange for either the PQF and/or the PSM Audit.

2. Contractors “self nominates” for the program.

3. Contractors must have worked a minimum of 5000 man-hours at an LAIA facility. Hours can be aggregated for work at multiple locations. Click here for a listing of LAIA facilities

4. OSHA & EMR data must be drawn from work performed in area facilities, not corporate data. If local data is not available, a detailed memo which defines how the data was derived must be placed in the presentation booklet which is submitted with the nomination form. The program is based on the most recent calendar year but reviews a 36 month “look back”. OSHA – EMR data reflecting that time period must be included.

5. Applicants must identify the type of work and category of man-hours they are applying for. Contractors can apply for both Specialty and General Contractor work, but a separate nomination form must be completed for each.

General Contractors:

Category I – 5000 to 100,000 man-hours annually
Category II – 100,001 to 500,000 man-hours annually
Category III – Greater than 500,000 man-hours annually

Specialty Contractors:

Category I – 5000 to 100,000 man-hours annually
Category II – 100,001 to 500,000 man-hours annually
Category III – Greater than 500,000 man-hours annually

6. Each applicant must have a program “safety review” audit which is conducted by a Safety Council Auditor. There is no fee for this audit. The safety review audit will review your nomination form, PQF, written programs, best practices, program enhancements, compliance programs (such as Drug & Alcohol & Training) etc. The safety reviews are on a first come first served basis. Schedule your review as soon as possible at audits@safetyally.org

7. A presentation booklet must accompany the nomination form which contains at a minimum the following information indexed (the quality of the booklet is used to provide bonus points):

a. The completed nomination form.

b. Two letters supporting your nomination signed by a member of the Safety Staff(s) of the LAIA facility at which you worked during the application year.

c. Supporting documentation;
i. for your written safety program,
ii. your drug & alcohol program including which drugs and medications tested for - the cut off levels – frequency of testing (employment) - random testing program,
iii. worker compliance training,
iv. OSHA 300 log, (3 years)
v. EMR data, (3 years)
vi. management systems to support safety performance for your organization,
vii. methods for measuring performance (beyond OSHA statistics),
viii. accident investigation protocol
ix. other documentation which provides support for your answers provided in the nomination form.

8. Bonus points are awarded for “Best Practices” that the applicant has in place to enhance Safety Performance. The best practice program must be reviewed with the “Safety Review Auditor” for consideration.

9. Bonus points are also awarded for steps taken during the past year to improve safety performance. This is beyond the best practice category, this is for those steps taken by the contractor who has identified areas that needed additional support and addresses both enhancements and corrective measures.

10. Recognition by customers, OSHA VPP, organizations such as API or ABC all should be included in the booklet and will be considered for bonus points.

11. A local contact person must be attached including name, title, E-mail and phone number.
